  • The Will County Dental Van will be here once again on Thursday Jan 10th so complete the required paperwork and ensure your child’s dental exam now!

    Rockdale School has been working with the Will County Health Department Dental Van for many years now to provide dental services to our students. Best of all, services are provided right here at Rockdale School. That’s right, they come to us! The Health Department will send the Dental Van to the school and provide dental exams, cleanings, fluoride varnish, and sealants if needed. No travel, no appointment, and no cost to families. It doesn’t get much more convenient than this!

    This is a great opportunity to promote dental health to your children. It’s also a great opportunity for parents/guardians to get a head start on required dental exams for next school year or catch up on requirements not yet met for this school year.  If your child is currently in Kindergarten, 2nd, or 6th grade and has not yet provided a dental exam, this can fulfill that requirement. Also if your child will be going into 2nd or 6th grade in the 2019-2020 school year, they will require a dental exam. The dental exams that will be provided to students through the Will County Dental Van on January 10th will count towards this requirement for next school year.

    The packet that the Will County Health Department requires for these dental services was sent home with students on Friday November 30th. If additional paperwork is needed it can be picked up in the nurse’s office or printed from this website under health forms. This paperwork is REQUIRED to sign up for these dental services on January 10th. This paperwork MUST be returned to Rockdale School by December 21, before Christmas break.

     

     

     

    Welcome!

    Should students feel ill at any time during school, they must notify his/her teacher of the condition. The teacher will provide a pass to go to the office. The office will determine what should be done. No medicine will be dispensed without an "Administration of Medication at School" form filled out and signed by both the doctor and parent. Emergency first aid will be given when needed. If you are injured, notify the teacher immediately, so that necessary help can be given. In the event that an ambulance must be called for a student, the parents must assume the financial responsibility for the cost.

     

     

    Illinois School Health Exam Requirements

    for 2018-2019 School Year

     

    Kindergarten

    1. Physical Examination and Immunizations- on Illinois form completed within a year prior to the start of school 

    DTP/DTap- Four or more doses with the last dose qualifying as a booster and received on or after the 4th birthday

    Polio- Four or more doses of the same type of Polio vaccine with the last dose qualifying as booster and received on or after the 4th birthday

    Measles, Mumps, Rubella (MMR)- Two doses of MMR or two doses each of separate Measles, Mumps and Rubella vaccines

    Varicella (Chicken Pox)- Two doses of varicella vaccine or proof of having chicken pox

    Lead Screening Test-students living in zip code 60436 must have a BLOOD test to check for lead levels

    Diabetes Screening-All students must be screened for diabetes through a few questions asked by a medical provider

    2. Dental Exam- on an Illinois form completed by a dentist within 18 months prior to May 15th of the school year

    3. Vision Exam- on an Illinois form completed by an eye doctor within a year prior to the start of school

    2nd Grade

    1. Dental Exam- on an Illinois form completed by a dentist within 18 months prior to May 15th of the school year

    6th Grade

    1. Physical Examination and Immunizations- on Illinois form completed within a year prior to the start of school

    Tdap booster- for tetanus, diphtheria, and pertussis (whooping cough)

    Meningitis Vaccine- one dose received after the child's 10th birthday

    Diabetes Screening- All students must be screened for diabetes through a few questions asked by a medical provider

    2. Dental Exam- on an Illinois form completed by a dentist within 18 months prior to May 15th of the school year

     

    We ask that all required examinations are turned in prior to the first day of school. In compliance with Illinois state law, if immunizations and physical are not up to date and on file by Oct 15th your student will be excluded from school until immunizations and physical are up to date.
